In this privacy notice, we confirm what information we hold about you and how we are permitted to use it.

At JGPhysio we take the privacy of your personal data very seriously.

We collect and hold certain personal data when you telephone, text or email to make an enquiry or email. At your appointment we will ask and record your date of birth, address, GP contact details and your private medical insurance details. This information is taken to meet mandatory legal requirements regarding medical notes along with your medical history and a record of treatment and advice given. Your information will be used invoice your insurance book a clinic appointment or class. At the point of enquiry we may ask you for your full name and telephone number and company if applicable and correspond with other medical personnel as necessary All correspondence/documents are password protected and stored on a laptop and data and clinical information is held on paper records which are stored in a locked filing cabinet in the physiotherapist’s private residence. They are kept in the physiotherapist’s personal care at all times at the clinic or class and while she transports them to and from her home to the venue on the days you have an appointment or a class with her.

We also collect data to assist in the administration of the business to provide you an effective service. Your personal contact data will be used to assist with the administration of your appointments/changes to scheduled appointments and/or reminders about appointments and to correspond with other healthcare providers and medical insurance companies. Any email correspondence with personal data will be sent password protected. We may also use your data for feedback and audit purposes.

All your information has to be held for 8 years after the conclusion of treatment. If the record relates to a child or young person, records must be kept until the client’s 25th birthday.

You should be aware that as the client, you can modify/ change inaccurate or incomplete information that we hold relating to your personal data. You are able to access it, retract consent to hold and process your records and raise a complaint, should you wish to do so. In order to raise a complaint we ask that you put it in writing to
